Thank you for choosing us as your family dental provider. We look forward to providing you with optimal and high quality treatment at an affordable cost. We have some financial options available for your convenience.

Financial Options

1. You may choose to pay for the services at the time of the treatment. If you have insurance, you only pay your deductable and the estimated portion not covered by insurance. We accept payment with cash, check, money order, Visa, MasterCard or American Express. For procedures that take two appointments such as crowns, bridges and nightguards, you may elect to pay half of your balance at the end of your first appointment and the remaining half at the end of your second appointment.
2. If your total treatment plan is over $3,000 (in case of insurance coverage, if your estimated portion is over $3,000), you may elect to pay full amount with cash, check or money order to receive a 5% discount.
3. Financed monthly installments up to five years.
4. Interest - free financing, when paid in six to twelve monthly installments.
   

Invisalign Financial Options

1. Down payment of 30% of the estimated portion (in case of insurance coverage) or 30% of the entire amount, then convenient monthly payments on the balance.
2. Cash, check, or money order at the beginning to receive 5% discount.
3. Financed monthly installments up to five years.
4. Interest - free financing, when paid in six to twelve monthly installments.
   

Insurance benefits explanation

We work with most insurance companies. If you have insurance, we are happy to estimate your benefit and ask that you pay only your estimated portion as the services are rendered. Your estimated patient portion for services is based upon the information provided by your insurance company and it may not be the exact amount of your benefit. If your insurance pays more than their estimated portion, you will receive a refund check, if it pays less you will be billed for the balance. We will bill your insurance company as a courtesy to you. Please provide us your insurance card and the company’s full name, address, phone number and policy identification number. Please notify us of any changes of insurance information. Any balance not covered by your insurance is your responsibility. If you feel your insurance company has erred, you may contact them. Any balances past due over 60 days will be charged an interest rate of 1.5% per month.

Dental insurance is a contract between your employer and a dental insurance company. The benefits that you will receive are based on the terms of the contract that were negotiated between your employer and the dental insurance company. Some plans exclude some services such as orthodontics, while other plans cover more extended dental services. The services selected are based on the cost of the policy to your employer and the arrangements with the dental insurance company. Our treatment plans are based on each patient’s needs and not on whether treatment is covered by a dental plan. However, we work with your insurance company to obtain the maximum benefit coverage for your dental treatment. We do not compromise our treatment plan to accommodate insurance plan coverage but we are happy to discuss advantages and disadvantages of different treatment options to help you make the best decision based on your need.

Cancellation policy

When you schedule your appointment, this time is reserved especially for you. A considerable amount of planning and preparation goes into each patient’s appointment time. If you need to change your appointment, please give us 48 hours notice. No-shows or appointments cancelled with less than 48 hours notice are subject to a cancellation fee.
